Student Engagement Data Request Form

  • Strategic Planning, Assessment and Analysis is committed to reviewing and processing your request in a timely manner. SPAA works to process all complete and approved Student Engagement Data Requests within 5-7 business days.

    In order to expedite the processing of your request you will want to review the following and gather the necessary materials and documentation required to complete your submission. In addition to general information about your data needs, you may also need the following:

    • If your request is associated with an ImproveKSU Outcome, please identify which outcome as this will be required for your submission.
    • If your request is associated with an IRB approved research project, please have your IRB number available.
    • If you have submitted this request in the past, please locate information on who had previously processed your request.
    • A list of individuals who will have access to student-level data (if included in your request).
    • If student-level data is included in your request and this is your first Student Engagement Data Request of the fiscal year, you will need to locate and upload copies of the following:
      • Buckley Form (accessible via UITS Security Forms)
      • KSU Information Security Training Certificate of Completion (accessible via your Learning Transcript within OwlTrain).
    • Email address of your Department Head (your Student Engagement Data Request will be routed to your Department Head for review and approval prior to SPAA reviewing your request).

    Incomplete submissions will be returned to the requester and will result in a delayed processing time.

    SPAA's processing timeline begins once your request has been approved by your Department Head.

Owl Life Designee to SPAA Form

  • Strategic Planning, Assessment, and Analysis (SPAA), under the direction of the Senior Associate Vice President of Student Affairs, has been charged with administering and maintaining Owl Life on behalf of the Division of Student Affairs. SPAA works closely with individual departments to help them achieve their goals and objectives within the system.

    Each department director/leader is responsible for designating an Owl Life Department Manager Designee. This individual, under the guidance of SPAA, is responsible for upkeep of the department’s Owl Life page, will serve on a user groups committee, and will be SPAA’s contact for questions and outreach regarding the department’s Owl Life page. This individual will be responsible for creating and/or approving events within their department. Though the director/leader may choose to self-select this role, many times the director selects another staff member to fulfill this role.

Owl Life Community Access Request Form

    In order to expedite the processing of your request you will want to review the following and gather the necessary materials and documentation required to complete your submission:

    • If your request for Community Access includes access to student-level data, you will need to locate and upload a copy of your Buckley Form (accessible via UITS Security Forms).
    • Copy of KSU Information Security Training Certificate of Completion (accessible via your Learning Transcript within OwlTrain).
    • Email address of your Department Head and Senior Leadership Team member (your Student Engagement Data Request will be routed to your Department Head and SLT member for review and approval prior to SPAA reviewing your request).

    Incomplete submissions will be returned to the requester and will result in a delayed processing time.

    SPAA's processing timeline begins once your request has been approved by your Department Head and Senior Leadership Team member.
